Questionnaire Instructions
The questionnaire is easy to fill out. Some of the questions are repetitive, they are set up not to test your memory, but that they address different organ groups.
Do not spend too much time with each questions, if they pertain to you, please answer them with a (1) one, (2) two, or (3) three. If they have no relevance or you have no idea, please them blank, we can review them; just mark them with a question mark.
Answer a (1) if the problem is mild or if it occurs once in awhile, place a (2) if the problem is moderate or occurs more frequently, answer a (3) if the problem is severe or occurs more frequently or often.
